Children: To obtain the greatest benefit from our services, we request that your children do not accompany you to your appointment. Patients under the age of 18 will need consent signed by a parent or legal guardian.

Cancellation Policy: A 24 hour notice is required for all cancellations. A credit card or deposit may be necessary to hold your appointment. A $25 charge will be assessed when appointments that are not cancelled without a 24 hour notice.
